Non-professional referrals

  • The information collected here will enable us to contact the person, check their suitability, discuss what is involved and, with their consent, contact their GP to complete the referral process.

  • Completing this form does not guarantee a place on the Programme.

The criteria are:
  • >80 years; or >65 years Maori and Pacific people
  • Living independently in the community, not in rest home or hospital care.
  • At risk of falls, or had a fall in the last 12 months

Handspring delivers the Programme in the following areas:

Wellington, Hutt Valley, Porirua, Kapiti Coast, Horowhenua, Manawatu, Wanganui, Wairarapa, and Gisborne.

If the enquiry relates to someone in these areas we will contact them shortly.  If the enquiry relates to residents of other areas, your enquiry will be forwarded to ACC and passed to the local provider. If you have not heard anything from ACC or a local provider within a week, call 0800 844 657 and ask to speak with your local ACC Injury Prevention Consultant.
