Who can refer? What information is needed?
Medical Professional referrals
All referrals need to be completed by a medical professional. To request referral forms complete the enquiry form here. We accept medical referrals in the following formats:

- Healthlink form direct from MedTech or other PMS (preferred). Our EDI address is: willissp

- Post to: Handspring Limited, level 3, 150 Willis Street, Wellington 6011
The information we require includes a summary of the individual’s main health complaints, a list of medications, completion of the tick box entry criteria including consent confirmation, and contact details for individual and referrer. We should confirm your referral has been received within a working day.
Other Referrals
Self-referrals or non-medical professional community referrals are accepted. The process involves initially completing the expression of interest form.
We will contact the individual to discuss eligibility, what is involved in completing the programme and gain their consent. Next we contact their GP with the referral form, and if they deem it suitable they are asked to complete the form and send it to us for processing.
Depending on availability of the GP this process can take as little as a few hours.
What happens next?
Once the referral is received, it is screened to ensure it meets the entry criteria and that all necessary information is present. If accepted the Participant will receive a letter confirming this and that a provider will contact them shortly to arrange a suitable time for an initial visit. The letter will also remind the Participant what the programme involves and what is expected of them.
The time between receiving this letter and the first visit can vary. Providers are organising schedules across a wide region and may be waiting for a few referrals located in close proximity. If the delay is more than 4 weeks the Provider will call to inform the Participant.
